Foodwise for Life

Welcome to the Foodwise for Life mobile application (the App). The App is intended to support people to achieve and maintain a healthy weight. It includes advice and guidance on how to achieve a healthy balanced diet and techniques to help you to make behaviour changes that are right for you. The App has 6 sections with interactive games, quizzes, activities and tools that you will be able to view at your own pace. There is a short survey to answer at the end of the App for you to provide feedback.

This Privacy Notice provides information on how Cardiff and Vale University Health Board (“UHB”) collects and processes your personal data when you use the App.

Why will I be asked to provide information in the App?

The information you provide in the App, for example, star ratings and feedback comments can help us to understand how it works for participants. This can help us to know what is going well and what we can do to make it better.

What will happen to the information I give?

The information you provide will be used with the feedback from others to help us understand more about peoples experience of using the App. This will help us to decide if it is a good idea to keep providing it for people to support healthy lifestyles.

Will my taking part be confidential and will my personal details be secure?

All information which you provide will be confidential. The handling and storage of personal information will be covered by strict NHS data protection standards. We will aim to use the information we get from the survey answers to write a report which anyone can read. No one reading this report will be able to identify you.

How will my personal data be processed?

The UHB is the responsible data controller for any personal data submitted.
Data collected will be used for the purpose of evaluation of the Foodwise for Life App
Celf Creative Design Ltd has been enlisted as a data processor to host the personal data processed via this app on behalf of the UHB
The lawful bases for processing this personal data under the UK GDPR and Data Protection Act 2018 are:

UK GDPR, Article 6(1)(a): your consent
UK GDPR, Article 9(2)(a): your explicit consent

You have the Right to Withdraw your consent at any time, without affecting the lawfulness of processing based on consent before its withdrawal.
Data will be retained in line with the Records Management Code of Practice for Health and Social Care 2022 (Wales)
